New BIP32 structure

Summary:

In an email conversation between Thomas Voegtlin and Pavol Rusnak on March 27, 2014, Voegtlin suggested a possible optimization for looking at the first address of each account instead of the first ten. However, Rusnak argued that adding unnecessary logic would not decrease the number of required requests between a client and a server. This is especially true when the backend sends responses in "bulks" of 10 addresses or more.
